- This role will be embedded as part of a multi-disciplinary Geriatric Outreach Service (GOS), supporting transitions of care to Residential Aged Care Facilities
About the role
We are seeking a dedicated and highly skilled pharmacy professional to join our team, responsible for the organisation, management, and daily operation of a designated pharmacy area. In this pivotal role, you will provide a comprehensive and high-level range of pharmacy services, exercising independent judgement to deliver expert medicines information and therapeutic advice to patients, medical and nursing staff, and other health professionals. Your work will be guided by hospital and divisional policies, procedures, and the highest professional standards of pharmacy practice.
You will play a key role in optimising patient outcomes through the safe, effective, and cost-conscious use of pharmaceuticals. By implementing strategies that promote the quality use of medicines, you will contribute to the delivery of best-practice care across the hospital. You are a proactive and experienced pharmacy professional with a passion for delivering high-quality services, leading teams, and improving patient outcomes. Your technical expertise, leadership skills, and collaborative approach ensure you thrive in dynamic healthcare environments.

You have excellent communication, public relations, and problem-solving skills, with the ability to work independently and meet tight deadlines with accuracy.

You bring extensive experience in hospital pharmacy practice, along with proven leadership, supervisory, and service improvement capabilities.

You demonstrate strong research and data analysis skills, with the ability to implement strategies that enhance pharmacy service delivery.

You are professionally and technically competent in pharmacy practice, with comprehensive knowledge of relevant codes, standards, and multidisciplinary teamwork.
