Lead medication safety and quality in the delivery of health care by maintaining and evaluating safety and quality practices and initiatives.

▪ This role reports professionally to the acute care and patient f low team pharmacist leader.
▪ This position may supervise registered and intern pharmacists, pharmacy students and pharmacy assistants
Your key responsibilitiesClinical / Technical
- Deliver high level clinical review services as part of a multidisciplinary team in developing pharmaceutical care plans and monitoring patient outcomes across all specialties for older persons presenting to the PAH ED and GEDI outpatients of the PAH.
- Manage an allocated workload to deliver clinical pharmacy services, including assessment, intervention and evaluation within a multidisciplinary team
- Deliver Home Medicines review for high-risk patients identified by a member of the PAH GEDI team.
- Apply professional judgement to complex caseloads, exercising independent decisions within the boundaries of legislation, Queensland Health policy and local service standards.
Education and Training / Quality Improvement
- Develop and deliver high level presentations to other health professionals within and outside the hospital e.g. senior hospital staff, general practitioners, university students and community -based support groups and target client groups.
- Actively participate in collaborative interdisciplinary safety, quality and research initiatives as recognised as a point of reference within the project team
- Participate in multidisciplinary planning teams for the provision of medication management within the GEDI project and the PAH as well as participate in support networks relevant to the position for other Queensland Health facilities.
- Minimise risk by reviewing Pharmacy procedures and processes, identifying areas for improvement
Leadership/Management
- Implement appropriate liaison systems to ensure continuity of care upon admission and discharge including but not limited to direct liaison with community-based health care providers, general practitioners, Residential Aged Care facilities and be able to identify support organisations, their roles and available services.
- Provide input into planning committees for the development and improvement of the GEDI service at the PAH.
- This position may supervise registered and intern pharmacists, pharmacy students and pharmacy assistants.
- Maintain data quality and manage all information in accordance with legislation, standards, policies and procedures.
- Conduct patient clinical reviews and document appropriate notes in the patient integrated electronic medical record (or other suitable digital patient record). These will also need to ensure evidence of clinical activity for which activity-based funding may be allocated is maintained.
- Ensure effective counselling and medication management strategies are adhered to and document appropriate notes in the patient integrated electronic medical record (or other suitable digital patient record).
▪ Vaccine Preventable Disease (VPD): It is a condition of employment for this role for the employee to be, and
remain, vaccinated against the following vaccine preventable diseases during their employment (Health
Employment Directive No. 01/16):

o hepatitis B
o tuberculosis
- Pharmacy Possession of a Bachelor of Pharmacy Degree (or equivalent) f rom a recognised tertiary institution. Registration as a pharmacist with the Australian Health Practitioner Regulation Agency (AHPRA). While not mandatory, for HP4 roles a post graduate qualif ication (diploma or similar) or Advanced Pharmacy Practice level 1 or above or the completion or commencement of an AdPhA accredited Advanced Training Residency in a relevant specialty area would be well regarded.
- Rostered overtime and participation in on-call are a compulsory requirement of the role
- Variable working hours (between 7am and 8pm) and rostered working days across 7 days (including weekends) is a requirement of this role.
