About the Role
The primary purpose of this role is to provide Case Management and Care Coordination to private customers and Home Care Package recipients across all package levels. The Care Consultant will assess the ageing related needs of the customers and facilitate necessary supports and services within the limits of their individual home care budget and the scope of the funded program to ensure that customers can live safely and independently in their own homes.
This role is responsible for intake, initial and ongoing assessments, initial and ongoing service planning, case management, goal directed care planning, package spend, advocacy and regular/ongoing home visits and monitoring to assist Baptcare customers to age well in place.
Care and services are delivered utilizing a Consumer Directed Care Model ensuring flexibility and choice in the supports that Baptcare customers’ access.
Key Responsibilities
Manage a dedicated case load of consumers ensuring care needs and quality services are delivered in accordance with the Aged
Care Act (Quality of Care Principles) and the Home Care Package guidelines, including the following responsibilities:
- Complete Initial Assessment with the consumer in their home, identifying care needs and safety related supports that will enable the consumer to remain living as independently as possible in their own home
- Based on assessment conducted, create careplans with consumer for services and supports. Careplans must be based on identified consumer goals and any risks identified through the consumer assessment
- Conduct regular home visits with consumers to determine changed needs due to a change in consumer circumstances or deteriorating health
- Conduct regular evaluation of care plans to ensure supports and services remain relevant and address new care requirements due to a change in consumer circumstances or deteriorating health.
- Support the consumer to make decisions regarding their care and services through the provision of information and through a dignity of risk framework
- Ensure that Home Safety Assessment is completed for each consumer and remains current, addressing any risk due to the consumer’s home environment, so that services and supports can be delivered safely.
- In case of deteriorating of health of consumers ensure prompt referrals, care coordination with medical professionals ensuring the appropriate interventions are in place to support the consumer and to prevent further deterioration.
- Ensure consumer vulnerability is identified at initial assessment and in ongoing assessments, and that appropriate care planning is in place to address vulnerability ensuring the consumer can remain living in their home safely.
- Escalate concerns regarding client safety if risks cannot be mitigated
- Assess and action interventions such as need for guardianship if consumer lacks insight and decision making due to cognitive changes/decline.
Key Selection Criteria
- Demonstrated skills in case management
- Proven contemporary knowledge of consumer directed care and solid understanding of service delivery requirements to meet high/complex care needs of customers.
- Demonstrated ability to follow HCP guidelines and Aged Care Quality and Safety standards.
- Strong communication skills and capacity to maintain the personal dignity and confidentiality of all customers and their representatives, ensuring that all interactions demonstrate respect and compassion according to Baptcare’s values.
- An understanding of the issues experienced by Older Australians and their carers in the home and their needs e.g. Social, Spiritual and Cultural
- A positive and proactive approach to working collaboratively within a shared care team and with the broader community
- Experienced user of customer management systems, Microsoft Office (Excel and Word) and Microsoft Outlook applications.

Essential Qualifications:
- Tertiary qualification in Nursing, Allied Health or Social Work/Social Services related

- Satisfactory National Crime Check, renewed every three (3) years
- Driver’s licence and own reliable vehicle
- Full and ongoing Right to Work in Australia
