About the role
We are seeking a values-driven Early Intervention Case Manager, This role provides immediate support to individuals at risk of losing housing and assists with rapid rehousing, collaborating with internal/external partners and community groups to develop prevention and early intervention programs. Reporting to the Early Intervention & Assertive Outreach manger this is a Permanent, Full Time role, located in Surry Hills, NSW.
You will successfully
• Provide client-centred case management, supporting individuals at risk of homelessness to maintain housing or secure rapid rehousing opportunities.
• Apply Trauma-Informed Care, Housing First principles, and TSA’s No Wrong Door Policy in all case management practices.
• Build and maintain strong relationships with internal and external stakeholders, and deliver training on homelessness prevention.
• Ensure accurate record keeping, timely reporting, and adherence to legislative and organisational requirements.
• Mentorship: Build meaningful relationships with youth, serving as a positive role model and providing guidance and support.
• Support: Provide emotional and practical support to youth facing challenges such as academic struggles, family issues, or social isolation.
You will have
• Tertiary or other qualifications within community services, social work, or related discipline (Minimum Diploma Level)
• Demonstrated knowledge in early intervention case management
• Experience of working with an inclusive practice, including with people from culturally and linguistically diverse backgrounds, LGBTQIA+, women, men & families is highly desirable
• A current working with children’s check
• A current drivers license
• National police check

Conditions
Compensation is in accordance with SCHADS Crisis Assistance and Supported Housing Lvl 2
