Role description
This position is based within the multidisciplinary Ambulatory Care team at Dhelkaya Health. The Ambulatory Care team provides high quality, person-centred assessment and management for people in the community requiring rehabilitation from illness and/or life changing health events. It includes prevention of health deterioration, and reablement of people with acute, chronic and complex health conditions in order that they live their best possible life in the community.
Social work is practiced across the organisation in community based, sub-acute, acute and residential aged care settings. Social workers are specialist clinical, community practitioners and multidisciplinary team members. Working within human rights and the social model of health, social workers address biopsychosocial issues to build better individual and community wellbeing.
The position contributes a biopsychosocial perspective to care planning, and identifies risks, social resources and strengths which contribute to better health. The social worker works within a multidisciplinary team providing care planning and short-term intervention, in a range of clinics, including neurological rehabilitation, cancer rehabilitation and survivorship and falls and balance. The social worker will undertake assessments, work with clients, carers, and internal and external agencies to enable improved health outcomes.
About you
* Tertiary degree in social work and eligibility for membership with the Australian Association of Social Workers (AASW).
* A minimum of five years' experience as a social worker in relevant practice areas.
