About the role
We are seeking a highly motivated Care Consultant to join our team that can visit clients in their homes: located in the Warrnambool, Glenelg, Colac-Otway, Moyne Shire

As a Care Consultant you will assess the ageing related needs of the customers and facilitate necessary supports and services within the limits of their individual home care budget and the scope of the funded program to ensure that customers can live safely and independently in their own homes. This role is responsible for intake, initial and ongoing assessments, initial and ongoing service planning, case management, goal directed care planning, package spend, advocacy and regular/ongoing home visits and monitoring to assist Baptcare customers to age well in place.

About you

To be successful in this role, you will need the following:

  • Qualification in Case Management, or
  • Undergrad qualification in Nursing / Allied Health / Social Work is essential
  • Div 1 Nursing Qualification - desirable but not essential
  • Prior case management experience will be highly regarded
  • Experience completing initial and annual customer assessments and care planning
  • Knowledge of the Home Care Package industry
  • Must be willing to undertake a police check
