Essential Requirements
- This is an identified Aboriginal Health position. Applicants must be a person of Aboriginal and/or Torres Strait Islander descent. An applicant’s race is a genuine occupational qualification and is authorised under section 14 of the Antidiscrimination Act 1977 NSW. Aboriginal applicants must demonstrate Aboriginality in addition to addressing the selection criter
- Bachelor degree or higher level relative to role (including Bachelor Health Science (Mental Health) for Djirruwang Program graduates.
- For Allied Health staff – Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) for Occupational Therapy or Psychology professions, or eligibility for membership of Australian Association of Social Workers (AASW) for Social Work.
- For Nursing staff – Current unrestricted authority to practice as a registered nurse with the Nursing and Midwifery Board of Australia and who has at least three years full time equivalent post registration experience in mental health, and approved post registration tertiary qualification in mental health.
- The CTT is a statewide service, supporting people from several correctional centres across Sydney metropolitan and regional NSW, and liaising with all NSW Health Local Health Districts. There is a requirement to travel to various locations across NSW, on occasion.
The Role
The Aboriginal Mental Health Clinician (Community Transitions) will support Aboriginal and Torres Strait Islander people who experience severe mental illness to access the necessary care and support required to ensure a successful transition from custody to the community. The Aboriginal Mental Health Clinician (Community Transitions) will be part of a multidisciplinary team that works in partnership with the Custodial Mental Health Service, primary health networks, Community Mental Health Services and other government and non-government organisations (NGOs) to provide a collaborative approach to ensuring continuity of care. By understanding patients’ clinical, psychosocial and behavioural vulnerabilities, and social and emotional wellbeing needs, the Aboriginal Mental Health Clinician (Community Transitions) will utilise general case management and discipline specific skills to ensure appropriate access to mental health care and cultural and psychosocial supports for Aboriginal people with serious and enduring mental health needs as they transition out of custody and return to or reconnect with their community.
The Community Transitions Team is one of the NSW Community Forensic Mental Health Services. The team is community based; but provides prison in-reach services to support people’s transitions back into the community. Its aim is to develop mental health care plans, establish necessary therapeutic and cultural supports, and provide follow-up community care to people leaving custody. The team is recovery-oriented and promotes positive health outcomes and community participation.
PLEASE NOTE: Hybrid role based at Sydney Olympic Park on Wangal land and working across multiple sites in custody and flexible work from home arrangements

The Justice Health Olympic Park (JHOP) office is located at 10 Herb Elliot Avenue, Sydney Olympic Park. Designed for inter-departmental communication and collaboration, the office provides large open spaces and an abundance of natural light. An array of restaurants and cafés are a short walk away.
Public transport is readily available including buses, trains, and ferries. Easy road access is provided via the M4 and Parramatta Road via Homebush Bay Drive. 35 km of bicycle paths surround the site and both bicycle storage and shower amenities are available inside.
