About the facility
Our major sites include University Hospital Geelong, in Geelong's CBD, the McKeller Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.
The Connected Care Program aims to deliver better health outcomes and reduce hospitalisation rates by improved integration of care with existing GPs, providing specialist assessment, negotiated, comprehensive care pland and early escalation pathways for clients. The service provides targeted and individualised service to consumers with complex chornic health or psychosocial needs at risk or recurrent ED presentations, admissions and readmissions. The service is primarily delivered in the home, with telehealth, phone or in-clinic appointments at Barwon Health facilitites as required.
About the role
Connected Care Administration is:
- responsbile for consumer focused adminsitrative support for a large multidisciplinary and dual site program
- being a central point of contact for consumers, families/carers, internal and external service providers to access the Connected Care team
- to facilitate the electronic registration and closure of Connected Care referrals
- to support site administration to enable the Connected Care team to be as effecicient and effective to acheive desired outcomes
- previous health administration experience
- demonstrated ability to communicate with people at all levels and varying communication styles
- the ability to work in a large multidisciplinary team
- a proven ability to multitask, manage competing priorities and problem solve
- proven computer literacy and ability to learn specialist programs
