What your day will look like….

As a Nurse Unit Manager/Team Leader for the Transition Care Program, your day will involve leading a multidisciplinary team, ensuring high-quality, coordinated care for patients' post-hospital discharge. You'll embrace and model Darling Downs Health values, oversee clinical governance, and manage HR and financial delegations. Additionally, you'll support a positive workplace culture, lead service planning and quality initiatives, and collaborate with various stakeholders to achieve optimal health outcomes.
