Position Summary

As the Head of Care Services, you will lead and manage the delivery of care across Fortify’s service offerings. This role oversees operational functions, clinical governance, compliance, and NDIS systems. With a focus on strategic growth and service quality, you will ensure the implementation of a tiered care model that is responsive, sustainable, and person-centred.

Key Responsibilities

  • Operational Leadership
  • Lead and manage care service operations ensuring NDIS compliance and efficient delivery.
  • Set, monitor, and review service objectives and performance targets
  • Identify and mitigate risks affecting stakeholders, finances, and organisational reputation.

Clinical Support

  • Oversee care plans, including mental health and chronic disease management.
  • Promptly escalate clinical issues to the Clinical Nurse Consultant (CNC).

NDIS System Management

  • Ensure effective implementation and maintenance of NDIS systems.
  • Drive continuous improvement, training, and resource allocation.
  • Communicate policies and responsibilities across teams and stakeholders.
  • Ensure adherence to Fortify’s policies, procedures, and regulatory frameworks
  • Promote a safe workplace by reporting hazards and maintaining confidentiality.

Customer Service and Supervision

  • Foster strong relationships with clients, staff, and external stakeholders.
  • Supervise and support care staff and contribute to team planning and performance.

Team Engagement & Development

  • Promote a positive team culture and model Fortify’s values and code of conduct.
  • Participate in training, team-building, and continuous improvement initiatives.

Workplace Health and Safety

  • Uphold WHS policies and ensure personal and team safety.
  • Report and manage risks, wear protective equipment, and participate in rehabilitation when required.

Selection Criteria

  • Skills & Experience
  • Proven experience as Head of Care or Care Manager.
  • Minimum 3 years’ management experience in NDIS services.
  • Strong understanding of NDIS standards and clinical governance.
  • Demonstrated leadership, compliance oversight, and risk management skills.
  • Excellent communication, interpersonal, and organisational abilities.
  • Current QLD driver’s licence.
  • Valid Paid Blue Card or willingness to obtain one.
  • NDIS Worker Screening Check or eligibility to obtain one.
  • Legal right to work in Australia.
