About The Role
The Clinician – Care Department is a senior clinical role responsible for providing clinical supervision and guidance to team leaders and care partners who deliver care management within the home care sector. This position ensures that team leaders and care partners are empowered to provide high-quality, consumer-centered care while adhering to clinical standards, regulatory requirements and organisational policies. The Clinician will be an integral part of the leadership team, supporting the professional development of team leaders and care partners, ensuring best practices in care management, and contributing to the continuous improvement of service delivery.
The Clinician will ensure that team leaders and care partners are equipped to effectively manage care plans, escalate clinical concerns as necessary, and maintain a focus on safety, compliance and client satisfaction.
Role Responsibilities
- Clinical Supervision & Risk Management: Provide expert clinical supervision and support to team leaders and care partners, ensuring care is person-centered, safe, and aligned with best practice and regulatory requirements. Act as the escalation point for complex clinical issues, offering practical guidance and strategies for effective decision-making and risk management.
- Incident Management: Lead timely and compliant incident responses in collaboration with the Clinical Governance Team. Support teams in reporting, investigating, and learning from incidents in line with SIRS and Trilogy Care’s policies, with a strong focus on continuous improvement and client safety.
- Professional Development & Training: Deliver ongoing training, mentorship and clinical guidance to enhance the skills and confidence of care teams. Promote professional growth and clinical excellence through structured development opportunities, reflective practice, and resource sharing.
- Compliance & Quality Assurance: Ensure all clinical practices comply with the Aged Care Act 1997, Aged Care Quality Standards, and Home Care Package guidelines. Provide expert input into audits, quality improvement initiatives and clinical policy development.
- Collaboration & Stakeholder Engagement: Build strong, collaborative relationships with internal teams, external providers, and other health professionals to ensure integrated, high-quality care. Contribute clinical insights in multidisciplinary forums and ensure alignment between care teams and organisational goals.
- Client-Centered Clinical Support: Ensure care plans are clinically sound, tailored to individual client goals, and regularly reviewed for effectiveness. Support teams in delivering responsive care that promotes client wellbeing, independence, and dignity.
- Leadership & Innovation in Care Management: Champion clinical excellence and innovation by identifying opportunities for improvement, implementing evidence-based practices, and contributing to strategic clinical initiatives within Trilogy Care.
Attributes and Qualifications
- Tertiary qualification in healthcare (e.g., Nursing, Allied Health, or a related field)
- Current, unrestricted AHPRA registration with the relevant governing Board
- Minimum of 2 years’ clinical experience, ideally within the aged care or home care sector
- Experience supervising or mentoring clinical and/or care teams in home care settings (preferred)
- Demonstrated ability to deliver high-quality clinical care and manage complex cases
- Strong clinical assessment and sound decision-making skills
- Excellent communication skills, with the ability to provide clear, constructive guidance and feedback
- Proven leadership and mentoring capabilities to support team development
- In-depth understanding of aged care legislation, quality standards, and clinical best practices
- Ability to analyse and interpret clinical data to improve care outcomes
- Advanced problem-solving and conflict resolution abilities

Mandatory Requirements
- Current federal police clearance (not more than 12 months old) and international police check if you have lived overseas in the last 10 years
