Job Overview:
As the Community Leader, you will take the lead in delivering high-quality community participation and social activity services for adults with disabilities. Reporting to the Manager Home and Community, you will be responsible for ensuring services are tailored, rights-driven, and aligned with participant aspirations.
In this role, you will collaborate with clients, their families, and support workers to develop and implement individualized plans that enable clients to achieve their goals. A key focus will be ensuring service quality, promoting continuous improvement, and fostering a positive, inclusive team culture. You will also contribute to Mosaic's strategic objectives, drive growth, and uphold compliance with the NDIS Quality and Safeguards Commission standards.
Key Responsibilities
- Lead the service delivery of client-focused, person-centred services that align with NDIS standards.
- Collaborate with clients and families to develop tailored support plans that reflect individual goals and aspirations.
- Actively manage and grow community participation and social activity services, ensuring high-quality outcomes of the service area
- Mentor and supervise a team of support workers, fostering a culture of empowerment, collaboration, and excellence.
- Drive compliance with regulatory standards, including restrictive practices and behaviour support rules.
- Create opportunities for continuous quality improvement and innovation in service delivery.
- Advocate for clients, providing professional support during NDIS planning and review meetings.
- Provide input into the development of the annual budget for the relevant area and ensure that resources are used effectively within the agreed parameters as budgeted
- Oversee workforce planning, participate in the processes of recruitment, onboarding, and ongoing staff development.
- Mentor, coach and supervise staff within the team and collaborate with People and Culture Team for the performance management including annual performance reviews,
- Promote safety, inclusivity, and cultural awareness across all interactions.
- Comply with, promote and assist to continually improve Mosaic’s policies, processes, and procedures related to Client Services.
- Provide after-hours on-call support on a rostered basis.
- Perform other duties and responsibilities relevant to the role as directed in the position description
To be successful in this position you will have:
- Relevant qualifications and/or experience within Disability or Community Service
- Minimum three years' experience working in the Disability/Aged Care sector is essential.
- Experience in successfully leading teams across multiple service offerings
- Experience in working with clients with challenging behaviours.
- Demonstrated understanding of client focused and person-centred support practices.
- Demonstrated commitment to a safety culture and inclusivity
- Ability to prioritize and excellent time management skills
- Agile and adaptable to changing circumstances
- Ability to work independently and collaborate with other teams for achieving common goals.
- High-level communication, negotiation, advocacy and interpersonal skills in order to build good relationships and trust with colleagues, clients, their families and guardians and other relevant stakeholders.
- Excellent written communication skills.
- Problem solving and decision-making skills with rationalized approach
Conditional employment requirements:
- Unrestricted working rights in Australia.
- Current Western Australian Driver's License.
- Licensed and Insured Vehicle
- Valid NDIS Worker Screening Check or willing to get one upon request.
- Completed NDIS Worker Orientation Module or willing to complete it upon request.
