Our client is a well-established community organisation that focuses on providing culturally appropriate aged care and community support services, particularly to the Asian community in the Perth metro region. As a not-for-profit entity, the organisation’s primary goal is to service the community through services provided such as home care packages, social support programs, and other aged care services tailored to the diverse cohorts.

About the Role
Our client is seeking to appoint a passionate and community focused Care Partner to join the team of linguistically talented professionals.
In this position, the successful candidate will be responsible for cultivating a collaborative environment with clients, families and multidisciplinary teams to ensure the delivery of consistently high standards of service.
Duties will include supporting a large number of Cantonese-only speaking clients by identifying funding requirements, performing assessments and creating care plans. In addition the Care Partner is required to review and adjust plans based on feedback from clients, family members and support workers.
It is essential that the Care Partner liaises with clients, support workers, allied health professionals and family members by using their proficient knowledge of the Cantonese language and culture.
The role will based primarily on site in the Perth CBD office, with 3-4 home visits per week.

To be successful you will have:
  • A relevant Allied Health (Physiotherapy, Occupational Therapy, Social Work, Registered/Enrolled Nurse), Health Science or Public Health qualification
  • Proficiency in speaking Cantonese
  • A current and valid driver’s licence
  • Excellent rapport building abilities especially with aged care and vulnerable people
  • Passion for helping people from varied backgrounds
  • Ideally, pervious experience in aged care, disability and/or working with vulnerable people
  • Strong time management and organisational skills
  • Ability to work flexibly and autonomously
  • Effective written and oral communication skills
  • Membership of peak bodies such as AHPRA, APA or OTA are considered desirable, but not essential
  • Ability to speak Mandarin in addition to Cantonese would be seen as highly desirable
  • First Aid Certificate, and a current (or ability to obtain) police check
